To ensure a smooth start to your child’s experience at KinderHaus, we kindly ask that the following documents be submitted before your child’s first day:
- Signed Parent Handbook
- Original Medical Record (valid for 1 year, must be updated annually)
- Completed Child Info Forms (for first-time enrollment)
- Signed Medical Treatment Consent Form
- If your child has any allergies, please also provide: a. A completed Allergy Action Plan
b. Individual Health Care Plan completed by your child’s pediatrician.
Submission Instructions:
- Mail or hand-deliver originals in an unfolded envelope
