Required Documents

To ensure a smooth start to your child’s experience at KinderHaus, we kindly ask that the following documents be submitted before your child’s first day:

• Signed Parent Handbook 
• Original Medical Record (valid for 1 year, must be updated annually)
• Completed Child Info Forms (for first-time enrollment)
• Signed Medical Treatment Consent Form
• If your child has any allergies, please also provide: a. A completed Allergy Action Plan b. Individual Health Care Plan completed by your child’s pediatrician.

Submission Instructions:

• Mail or hand-deliver originals in an unfolded envelope
• Email scanned versions to: info@kinderhausbrooklyn.com
